Facility Guidelines
Resident Policy at Founders Pavilion
- Visiting hours are 10:30am to 8:30pm. Family and friends are encouraged to visit during these hours. To ensure a minimum of disturbance to other residents, visits during other hours should be requested from the charge nurse on duty.
- All visitors are required to sign the "Visitors Book" in the Lobby.
- Children must be accompanied and supervised at all times by a parent or responsible adult.
- A sign-out book at the nursing station on each unit must be completed when taking a resident out of the facility for social activities, rides, shopping trips, dinner, etc.
- Day visits and overnight stays are allowed on a limited basis. Overnight visits must be requested from the Social Work Department twenty-four (24) hours in advance of the planned visit. Remember to complete the sign-out book at the nursing station on the respective unit when taking a resident out.
- Families and/or designated representatives are required to provide a minimum three-day supply of personal clothing (i.e., slippers, shoes, dresses, trousers/slacks, hose, shirts, sweaters, undergarments, etc.) and all preferred personal care items not furnished by the facility. Families and/or designated representatives are responsible for preventing excess items from accumulating in the resident's room by removing out-of-season/unusable clothing, decorations, magazines. Flowers and plants that are out-of-season, dead, wilted or diseased will be discarded. The facility reserves the right to remove articles from the room.
- The designated representative must take all personal clothing to Housekeeping for labeling on admission and as new clothes are added to their inventory.
- The resident and/or designated representative must complete an inventory of personal effects form on admission. The facility should be informed when items are removed.
- The facility reserves the right to purchase needed articles for the resident. The cost of the clothing will be billed to the resident's family or legal representative.
- Personal laundry services are provided for machine washable items only. Families and/or responsible parties are required to care for all clothing and/or articles which need to be dry cleaned or hand washed.
- A personal account may be established in the Business Office to enable the resident to have funds available. The facility will deposit any resident's personal funds in a non-interest bearing account.
- Personal items of sentimental value, expensive jewelry, and sums of money, should not be left with the resident. We encourage these items be taken home. Items of value may be stored in the safe in the Finance Office.
- Please make staff aware of food items brought in from home or restaurants.
- Residents are encouraged to go to the main dining room for lunch and dinner meals unless medically contraindicated.
- Only authorized staff members should remove trays from the food trucks. Family members who wish to assist with feeding must be observed for competency.
- Guest trays may be ordered from the Dietary Department. Guest trays are to be ordered before 10:30 am for lunch and 4:30 pm for dinner. There is a minimal fee charged for this service.
- All medications must be prescribed by the attending physician and given by professional nurses unless self-administered according to facility policies.
- The facility allows retention and use of possessions as space permits unless this infringes on the rights of other residents or is medically contraindicated.
No more than two items of furniture are allowed per resident. Televisions are permitted but are limited to a 19 inch portable or smaller. A sturdy stand is required. (No larger than 26 inches wide by 15 inches deep). Dressers permitted are limited to 20 inches wide by 16 inches deep or smaller. These items must be approved and tagged by Maintenance. They are to be brought in Monday-Friday 8:am-3:pm.
Items are not to be hung or taped on the walls except by
facility Maintenance Personnel. Bulletin boards are provided upon request.
If you are assigned to a semi-private room, we encourage roommates to share a TV. If sharing a TV is not feasible, we require that each resident in that particular room acquire earphone/headphone options. The resident or family member is responsible for arranging the television cable service. The charge for installation and the monthly service charge are your responsibility.
- Electric blankets, heating pads, hot water bottles, etc., are not allowed in the facility for any resident. No personal medical equipment is allowed except for the purpose of resident discharge teaching.
- The resident or family member is responsible for arranging the telephone service. The charge for installation and the monthly service charge are your responsibility.
- A public courtesy phone is available at a convenient location on each unit. You will be assisted, if necessary, by a staff member to make or receive telephone calls. Privacy will be provided.
- If you occupy a private room, the facility reserves the right to transfer you to another room temporarily if isolation is required for another resident.
- A family member or the responsible party must accompany a resident to a physician's office for vision/hearing exams, and for any other medical services outside the facility. Arrangements for transportation will be arranged by the facility, if required.
- Beautician and barber services are available at reasonable rates. Arrangements can be made by contacting the beautician or the Business Office staff.
