Founders Pavilion Social Work Department is responsible for coordinating admissions, providing social services to each resident admitted to the facility and for ensuring each resident discharged has a coordinated, appropriate, safe discharge plan.

Founders Pavilion is a 120 bed skilled nursing facility that provides short term rehabilitation stays and long term resident care.

During a resident's stay in Founders Pavilion, Social Work Services will be available to assist with any social, emotional, psychological, financial and family problems that might interfere with recovery or rehabilitation. The services that a resident might expect will include but not be limited to individual or group counseling and bereavement services.

Social Work assists the interdisciplinary team, the family and resident in the development of a discharge plan. Prior to discharge, Social Work will assist with referral for followup services in your home or assist with placement in an alternative level of care.
